So that we don't have to lose time in our first session going over forms and business details, I’m going ask you to fill in some forms prior to your first appointment.  Please read the instruction on the Forms Page (accessible by clicking here, or on Forms menu above). 

Please note that the instructions are slightly different for those who use insurance versus those who chose to pay privately.

Please notice that the contract for services form that you're asked to sign contains a fair amount of legal information that licensed psychologists are asked by the American Psychological Association to provide to clients. With this information, you will see the established rules on confidentiality, missed appointments, etc. My form reflects current best practices. Sorry it's boring reading.
